What it is
An Unshaven Hair Transplant is an FUE-based approach in which extensive donor-area shaving may be reduced or avoided. Depending on the technique, selected follicular units may be individually trimmed before extraction or long-hair follicular units may be harvested from an unshaven donor area.
The underlying harvesting method remains Follicular Unit Excision (FUE), where individual follicular units are removed using small punches and transplanted into the recipient area.
02
Where can Unshaven FUE help?
Pigmentation can appear in different patterns. Understanding the type, depth and likely trigger helps guide a more suitable treatment approach.
For selected hairline restoration where patients want to minimise visible changes to their existing hairstyle.
For carefully planned temple restoration requiring natural direction and discreet donor management.
May be considered where graft requirements are relatively focused and compatible with an unshaven approach.
May suit selected patients who prefer less visible preparation around the time of transplantation.
Minimal-shave approaches may be considered for selected women where maintaining hairstyle appearance is particularly important.
Longer surrounding hair can sometimes help conceal donor-area preparation during recovery. ISHRS also notes that patients with long hair who want to avoid donor shaving may sometimes consider FUT as an alternative.
WHO MAY NEED FURTHER ASSESSMENT
Certain hair and scalp factors may require Unshaven Hair Transplant to be delayed, adjusted or reviewed before treatment planning.
Large areas requiring extensive graft numbers
Weak or limited donor density
Rapidly progressing or undiagnosed hair loss
Active scalp infection or inflammation
Hair characteristics that make unshaven harvesting technically unsuitable
Suitability depends on donor density, hair length, graft requirements and how safely the follicles can be harvested without extensive shaving.

What to expect
Donor & Hairline Assessment
Your donor density, hair-loss pattern, hair length and restoration goals are reviewed before deciding whether an unshaven approach is appropriate.
Selective FUE Harvesting
Target follicular units may be individually trimmed or harvested while surrounding donor hair remains longer, depending on the selected unshaven technique.
Graft Placement & Recovery
Prepared grafts are placed according to the planned hairline, angle and direction while surrounding hair may help make the early recovery period less visually noticeable.
04
Physician note
Unshaven transplantation is about discretion—not compromising surgical planning simply to avoid shaving.
Long-hair or non-shaven FUE is technically more demanding than conventional shaven FUE. The priority should remain safe follicular harvesting, appropriate donor management and natural graft placement.

Questions
It is an FUE-based transplant approach designed to reduce or avoid extensive shaving of the donor area in appropriately selected patients
Unshaven transplantation generally uses FUE harvesting, but modifies how the donor hair is prepared before follicular-unit excision.
Not necessarily. Some techniques individually trim the target follicular units while leaving surrounding hair long. True long-hair FUE may harvest follicles without cutting the hair shaft short.
Not universally. It mainly offers a different approach to visible donor-area preparation. Conventional shaving can provide the surgeon with a clearer field for efficient and safe harvesting.
Yes. ISHRS describes long-hair unshaven FUE as technically demanding and notes the potential for increased follicle transection if not performed appropriately.
Yes. Because follicular units are still harvested using FUE, small extraction scars remain. Avoid describing the procedure as scarless.
It may be considered for selected women, especially when visible shaving is a concern, although the most appropriate harvesting technique depends on individual donor and restoration needs.
There is no fixed number. Graft numbers depend on donor density, recipient-area requirements and whether an unshaven technique remains practical for the planned procedure.
Natural appearance depends on appropriate hairline design and correct graft angle, depth and direction—not simply whether the hair was shaved.

Minimal visible shaving
Not every hair transplant can or should be performed without shaving.
DISCREET HAIR RESTORATION
Less visible preparation. The same need for precise planning.
Keeping surrounding hair longer can make the transplant process more discreet, but it also makes follicular harvesting more technically demanding.
At Averah, an unshaven approach should be considered only when it fits your donor density, hair characteristics, graft requirements and long-term restoration plan.
